Volunteering
Tottenham-Beeton District Minor Softball Association is run strictly by volunteers. Volunteers are always needed.
  1. Registration Consent: I, the aforementioned parent or guardian do hereby approve of the above mentioned registrant’s participation on a softball or tee ball team under the jurisdiction of the association known as Tottenham-Beeton District Minor Softball Association and agree that we (parent or guardian and registrant) shall abide by the rules and regulations set forth by said association. I hereby release Tottenham-Beeton District Minor Softball Association, its directors, and all those affiliated with the association and all those competing in any activities engaged by the association, from any and all claims, damages, or causes of action, however arising, which heretofore may have been or may hereafter be sustained by me/us. Further, I accept all responsibilities and liabilities which may occur due to or because of transportation. TBDMSA reserves the right to make changes in our division ages for the 2023 season. There will be no extra cost to the player. I have read and fully understand this form in its entirety and affix my signature in witness thereof:

Code of Conduct Consent: I have read and understand and accept the “TBDMSA Code of Conduct” I understand that violation of any one of these policies can result in the possible removal of my/our child(ren) or myself as a parent or volunteer from a game, the park in which games are played or from Tottenham-Beeton District Minor Softball Association entirely. By signing below, I recognize that it is my responsibility to discuss the “TBDMSA Code of Conduct” with my child(ren) and therefore sign with them or on their behalf.

  3. Photograph Policy and Consent: Photographs of Tottenham-Beeton District Minor Softball Association (TBDMSA) participants (players and coaches) will be taken and collected throughout the year during TBDMSA events (e.g. games, practices, tournaments, clinics, etc.). These photos may be published in TBDMSA printed materials or online social media (e.g. TBDMSA Players’ Handbooks,TBDMSA website,  Facebook page) and may be displayed at TBDMSA events (e.g. tournaments, year end banquet, registration, etc.) for the current and future seasons to promote the spirit and goals of the game and TBDMSA organization…good sportsmanship, skill, athleticism, friendship and fun.  I have read and understand the Photograph Policy and give permission for photographs of my child to be taken and used in TBDMSA materials and displays as noted above.
  4. TBDMSA Refund Policy:

    1.       Full refunds will be given up to the time that the registrant has been placed on a team list and has been so informed.

    2.       From the time that the registrant is placed on a team list and until the playing season has begun, refunds in the amount of 50% of the fee will be given.

    3.       Once the playing season is underway, no refund will be made unless special circumstances and approved by the Executive.


  5. Rowan’s Law:

    Under Rowan's Law, before any player can be registered with Tottenham-Beeton District Minor Softball Association, the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review one of the Concussion Education Resources provided by the Province of Ontario and must review the Player Code of Conduct.     RELEASE AND WAIVER OF LIABILITY

    INDEMNITY AGREEMENT

     

    IN CONSIDERATION of allowing me to participate in the programme, related events and activities of the Tottenham-Beeton District Minor Softball Association (TBDMSA)

     

    I WARRANT TO YOU THAT:

     

    1.     I am familiar with the risk of serious injury and death which any participant in this programme must assume, and

     

    2.     I believe that I am physically, emotionally and mentally able to participate in this programme, and that my equipment is mechanically fit for my use in this programme, and

     

    3.     I understand that all applicable rules for participation must be followed and that at all times the sole responsibility for personal safety remains with me, and

     

    4.     I will immediately remove myself from participation, and notify the nearest official, if at any time I sense or observe any unusual hazard or unsafe condition or if I feel that I have experienced any deterioration in my physical, emotional or mental fitness for continued participation in the programme.

     

    I UNDERSTAND AND AGREE, on behalf of myself, my heirs, assigns, personal representatives and next of kin, that my participation in this programme and execution of this document constitutes:

     

    1.     an unqualified ASSUMPTION OF ALL RISKS associated with participation in this programme by me even if arising from negligence, or gross negligence, including any compounding or aggravation of injuries caused by negligent rescue operations or procedures, of the programme organizer and any persons associated therewith or participating therein, and

     

    2.     a FULL AND FINAL RELEASE AND WAIVER OF LIABILTIY of the programme organizer and all persons and organizations associated with it and the programme including, without limiting the generality of the foregoing, its officers, directors, officials, agents and/or employees, other participants, sponsors, advertisers, owners and /or lessors of the premises used to conduct the programme, sanctioning bodies, medical or rescue personnel (the RELEASEES), of and from with the respect to all injury, disability, death or loss or damage to person or property whether arising from the negligence, or negligent rescue of or by the foregoing or otherwise, and

     

    3.     an UNDERSTANDING NOT TO SUE the RELEASEES for any loss, injury, costs or damages of any form or type, howsoever caused or arising, and whether directly or indirectly from the participation in this programme by me, and

     

    4.     an AGREEMENT TO INDEMNIFY, and to SAVE and HOLD HARMLESS the RELEASEES, and each of them, from any litigation expense, legal fees, liability, damage, award or cost, of any form or type whatsoever, they may incur due to any claim made against them or any one of them whether the claim is based on the negligence or the gross negligence of the RELEASEES or otherwise.

     

    I HAVE READ THIS DOCUMENT THOROUGHLY.

     

    I UNDERSTAND THAT THE RELEASEES ARE RELYING UPON MY WARRANTIES, ASSUMPTIONS, WAIVER AND RELEASE, UNDERTAKINGS AND AGREEMENTS WHEN ACCEPTING MY PARTICIPATION IN THIS PROGRAMME.

     

    I UNDERSTAND THAT BY SIGNING THIS DOCUMENT I GIVE UP SUBSTANTIAL LEGAL RIGHTS I WOULD OTHERWISE HAVE.

     

    I agree to the terms and conditions stated above VOLUNTARILY AND WITHOUT INDUCMENT.
